Add Health Verification information

As an employee, student, guest or other individual, you can now directly share your vaccination card or test result with your employer or organization, to avoid insecure sharing via email or unauthenticated online forms.

1. The Health Verification portal is accessible at lifesite.co/health-verification. To get started you'll need your company or organization's personalized Company Code.

Contact us at covid-support@lifesite.co to enable this.

2. Provide your personal information to verify yourself with your employer or organization.

• Required fields: First Name, Last Name, Date of Birth
• Consent to Share Information is also required

3. Choose whether you will be providing your vaccination card, a test result, declaring you have not been vaccinated, or declining to share your vaccination status

4. Provide information contained on your vaccination card.

• Photo or scan of vaccination card: This is recommended but optional.
• Required fields: Manufacturer, Doses, and Dosage Date

5. Review the information you have provided. Be sure to confirm the information you have provided is accurate.

6. You're done!
