Once receiving your test kits, they need to be activated so we know which employee received which test kit. Test kits can be activated individually using the activation form or in a group using a .csv. For full instructions regarding the activation process, email covid-support@lifesite.co
.CSV Requirements
Example Template
When activating test kits through .csv, please follow the below requirements:
Headers: Required headers are: First Name, Last Name, Date of Birth (mm/dd/yyyy), Provider Code, Consent, COV#. All fields are required. Headers should be Row#1 in the document. No additional information should be included above the header row.
Blank Cell: In a row, if any cell is blank, the entire row will be skipped and that person's test kit will not be activated.
.CSV Format: Save all spreadsheets as .csv format. .xls format is not accepted

One Tab: Keep all data on one tab. Multiple tabs are not supported.
Extra Information: In the .csv document, include required information only. This means that the first row should be the column headers followed by each row containing employee information only. Any additional information you need to notify us of can be sent to us separately in an email to covid-support@lifesite.co
About the Columns
Date of Birth: All birthdates must be in the MM/DD/YYYY format
- Correct: "02/03/1993"
- Incorrect: "Feb 10 1993", "2/3/93" , "2/3/1993"
COV#: COV# is the number associated with the patient's unique test kit. This is found on the test kit's sticker and the top right of the form. All COV#s are 7 digits only
- Correct: "1234567"
- Incorrect: "Did not test" , "COV#1234567" , "COV1234567" , "12345678" , "1234"
Consent: Confirm that each employee has provided consent for test kit activation indicated with a "Y" only in this column.
Provider Code: This is your company's unique ID. The provider code is the same for all of your employees. This may vary only if your company has multiple locations. To confirm what your provider code is, please contact covid-support@lifesite.co
FAQs
Q: What if I only need to activate a few employees and not the whole list?
A: You can send a .csv that only contains the individuals to be uploaded. A full employee list is not required.
Q: What if a couple of employees did not test this week?
A: You can leave the COV#s for these employees blank and they will be skipped for this week only. Please note that in this case, the COV# field must be completely blank. Filling this field with a space, "did not test," or any other text will cause an error for the whole employee list.
Q: Can I add extra columns or headers to separate information by things like the team, location, etc.?
A: Please limit the information in the .csv document to the required fields (First Name, Last Name, Date of Birth). If you traditionally use extra columns or headers to store additional team data, please remove this information before sending us the .csv.
Q: Why not .xls?
A: An .xls file sometimes saves hidden information like formatting details and hidden content that will cause errors when activating the test kits. To avoid these errors, please save all spreadsheets as .csv files and double-check the content before sending it.

Q: What if I want my employees to activate their test kits themselves?
A: Employees are able to activate their test kits directly in LifeSite using the Activation Form.
